A well stablished and highly regarded physician was censured by the Medical Board due to an unfortunate circumstance. The physician prescribed (appropriately) a pain medication for a patient who recently had extensive orthopedic surgery. The patient subsequently died due to an overdose. Autopsy and postmortem toxicology revealed the patient died of respiratory arrest due to the combined use of hydrocodone and heroin. Unbeknownst to the physician, the patient was a habitual user of illicit drugs. The Medical board found the physician “negligent” simply because he did not perform urine toxicology for “screening and monitoring” the use and prescription of a controlled substance.

We are obtaining urine sample form you in order to perform urine toxicology screening. We are required to obtain toxicology screening from our patients who are on prescription medications that are classified as “controlled”
This testing is often done on urine or saliva instead of blood. Many drugs will show up in a urine or saliva sample. And urine and saliva tests are usually easier to do than blood tests.
The purpose of this urine testing is to ensure therapy compliance. Please contact our staff with any questions or comments.

TRUST BUT VERIFY
“Trust but verify” is a good advice in many life situations. When administering opioids for chronic pain, verification is vital to successful therapy and patient safety. This is because studies show discrepancies between medical direction and actual medication-regimen adherence among patients on opioid therapy.
Physicians need to consider the risks of medications we prescribe. They are faced with the daunting task of controlling patients’ pain. However, they could be responsible for sickness or even death due to pain medications. Thus, it is a physician’s charge to use caution, prescribe medication judiciously and do what he or she can to ensure that the medications prescribed are used as safely as possible.
The best way to protect yourself from any legal action which arise from such issues is by establishing a protocol in your clinical practice which would clearly show concerted effort of clinically monitoring patients who are on narcotic pain medications. Obtaining regularly scheduled urine toxicology screening should be a big part of such protocol.
